Credentials Service
The Office maintains a credentials file where recommendation letters of students and alumni are kept. Upon your request, recommendation letters are forwarded to graduate schools and prospective employers. There is a modest fee for this service which is renewable every two years. Request Form to Have Credentials Forwarded to Graduate Schools :
If you have established a credentials file at our Office and wish to request recommendation letters to be forwarded to graduate schools only, please complete the following form.
On-line request of recommendation letter is for Graduate School(s) application only. Request for employment purposes should be made by directly contacting the Office.
Two Requirements to complete form:
- Last four digits of your Social Security Number.
- The address where the recommendation letter(s) is to be forwarded should be a Graduate Admission Office.
